Required UFSTP Documentation

 

Thank you for submitting your application for participation in the Uniform Food Safety Transportation Protocol (UFSTP). As indicated in the application itself, there are two other key steps you must take in order to be confirmed as a UFSTP signatory.

1. The Uniform Food Safety Transportation Protocol must be signed by the motor carrier applicant's owner, partner, officer or director and notarized. You can download or print a copy of the formal UFSTP document here. Although you should have read the Protocol before applying, it is important to review it very carefully because you are committing your company to following the practices that it dictates. However, nothing in the Protocol requires you to change any existing practices that already comply with its terms. To expedite your confirmation as a UFSTP signatory, please send the signed and notarized UFSTP to documents@ufstp.com or fax it to 205-449-2993. Also, we will need a hard copy mailed to us for our records. The address is TransComply, Attn: Avery Vise, 9340 Helena Road, Suite F-306, Birmingham, AL 35244 USA.

2. Your company will need to arrange for its insurer(s) to provide certificates of BI/PD insurance in an amount of not less than $750,000 and cargo insurance in an amount of not less than $100,000 to: TransComply, 9340 Helena Road, Suite F-306, Birmingham, AL 35244 USA; documents@ufstp.com; fax 205-449-2993.  TransComply must be listed as the Certificate Holder, and the certificate must come to us from the insurance agent or underwriter, not through your company. To expedite your confirmation as a UFSTP signatory, please have the insurer send the certificate to documents@ufstp.com or fax it to us at 205-449-2993. The signed and notarized UFSTP document and valid certificate(s) of insurance must be submitted before your company will be confirmed and publicized as a UFSTP signatory.
